Neighborhood Socioeconomic Disadvantage and 30-Day Rehospitalization: A Retrospective Cohort Study.

Among medicare patients discharged with congestive heart failure, pneumonia, or myocardial infarction the 30-day rehospitalization rate was 21%. This rate increased to 27% among patients living in the poorest 15% of neighborhoods.  (Ann Intern Med. 2014 Dec 2;161(11):765-774.)
